Our Host Homes offer a nurturing environment where individuals receive personalized care from trained providers, ensuring comfort and support.
Family Caregiver
Through the Family Caregiver model, family members can provide essential care, allowing individuals to thrive in their own homes with familiar support.
Comprehensive Services in the Family Home
This model is designed for individuals who live at home but would like a non-family member to assist with personal care, meals, finances, medical needs and community activities.
Independent Apartments
Designed for those with life skills, our Independent Apartments program offers minimal yet effective support, promoting independence and community engagement.

We work with some of the best Host Home Providers throughout Colorado. Host Home Providers will assist individuals receiving services on the DD waiver with personal care, meals, finances, medical needs and community activities. Real Care aims to match individuals in services with equipped and welcoming host home providers that are trained to meet each individual’s wants and needs. They offer consistent and reliable care in a variety of settings such as, wheelchair accessible, medical knowledge / Nurses, with or without kids, SOMB trained, crisis intervention, independent environments, etc.
Host Home Providers receive on-going training and support from our agency Nurses, Finance and Benefits Coordinator and Program Support Coordination Team.
Family Caregivers
Family Caregiver
The Family Caregiver Act allows family members to assist individuals receiving services on the DD waiver with personal care, meals, finances, medical needs and community activities. The intent of the family caregiver model is to provide greater service flexibility, expand provider choice and living options. This model allows an individual to receive services in their own home.
Support and Training
Family caregivers benefit from continuous training and support from our dedicated team, including agency nurses, finance and benefits coordinators, and program coordinators. This comprehensive support system ensures that caregivers have the resources and knowledge necessary to provide high-quality, personalized care. Our commitment to caregiver education and assistance reflects our dedication to fostering a nurturing and supportive environment for all individuals and their families.
Comprehensive Services in the Family Home
This model is designed for individuals who live at home but would like a non-family member to assist with personal care, meals, finances, medical needs and community activities. Hourly support is available to the individual while the rest of the time a family member provides natural support.
Providers receive on-going training and support from our agency Nurses, Finance and Benefits Coordinator and Program Coordination team.
Independent Apartments Program
This is designed to support individuals who have life skills and require minimal support in their home and in the community. Real Care ensures that individuals receiving services on the DD waiver are matched with friendly and competent Providers that can assist with money management, shopping, household tasks, personal care, community activities and/or overall health and wellness.
Individuals in this program and their Providers receive on-going support from our agency Nurses, Finance and Benefits Coordinator and Program Coordination team.
