How to Access Developmental Disabilities Services

Participate in a comprehensive needs assessment conducted by the CMA to evaluate your support requirements and develop an individualized service plan that addresses your specific needs and goals.

Step 3: Waiver Enrollment and Provider Selection
Work with your case manager to enroll in the appropriate Medicaid waiver program and select service providers like Real Care that align with your individualized plan and living situation preferences.
