Supported Community Connections (SCC) Program

We offer 1:1 community access service through the SCC Program. Real Care’s SCC Program allows for individuals who don’t fit traditional day program models or have extensive needs the opportunity to engage in meaningful activities within their community. The Program focuses on integrating into the community, developing skills, and having opportunities to make choices. Participants are encouraged to actively engage in choosing activities and schedule of their preference.

Individuals receiving services on the DD waiver can choose a provider of their preference or Real Care can help match with a friendly and well-trained Direct Support Professional (DSP).

DSPs receive on-going training and support from our agency Nurses, Finance and Benefits Coordinator and Program Coordination team.

Please note: the SCC program through Real Care is only available to those receiving residential services through us.

Nursing Support

Individuals with developmental disabilities can experience a multitude of benefits through at-home nursing and healthcare services. Real Care Nurses help to personalize care that not only caters to the unique needs of the individual but also ensures comfort, convenience, and quality assistance in the comfort of their own homes. Our highly qualified nurses help to educate and train providers / caregivers on each person’s medical care needs.

Our Program Coordination team can also assist with connecting individuals with Private Duty Nursing while staying on the DD waiver.

Respite Providers will temporarily assist individuals receiving services on the DD waiver with personal care, meals, finances, medical needs and community activities. Real Care aims to match individuals in services with equipped and welcoming Respite providers that are trained to meet each individual’s wants and needs. They offer consistent and reliable care in a variety of settings such as, wheelchair accessible, medical knowledge / Nurses, with or without kids, SOMB trained, crisis intervention, independent environments, etc.

Respite Providers receive training and support from our agency Nurses and Program Coordination team.

Respite can be hourly or full day, in or out of the home and on a preferred schedule. It is only available to individuals receiving residential services through Real Care.
