Connects individuals on DD waiver with trained Host Home Providers who offer personalized care, accommodations, and support within their homes.
FAMILY CAREGIVERS
Enables family members to provide comprehensive care and support to individuals on DD waiver within their own homes.
COMPREHENSIVE SERVICES IN FAMILY HOME
Provides non-family caregivers to assist individuals living at home with personal care and daily needs while family members offer natural support.
INDEPENDENT APARTMENTS
Supports individuals with life skills who require minimal assistance to live independently with help for household tasks, finances, and community engagement.

Provided for Current Residential Clients
SUPPORTED COMMUNITY CONNECTIONS (SCC)
Offers personalized 1:1 community integration services for DD waiver recipients who prefer alternatives to traditional day programs.
FINANCIAL SUPPORT
Manages finances for individuals receiving Host Home services, including Room and Board payments and Personal Need Funds.
NURSING SUPPORT
Delivers personalized healthcare through qualified nurses who provide training and education on specific medical care needs.
RESPITE
Provides temporary care and support through trained Respite providers on a schedule that works for individuals and their primary caregivers.
Contractor Opportunities
HOST HOME / RESPITE PROVIDERS
We are currently seeking caring, committed, honest, and responsible Host Home/Respite Providers to provide care and support to adults with intellectual and developmental disabilities in your home. These individuals may have challenging behaviors and/or medical needs.
INDEPENDENT CONTRACTORS
All Host Home Providers are independent contractors. We serve individuals throughout Colorado. Training is provided by the agency!
EXPERIENCE AND SKILLS
While experience with developmental disabilities is preferred, it is not mandatory. We value professionalism, patience, and effective communication skills.
Communication
Good written and verbal communication skills are essential. A valid driver’s license and a motor vehicle record are required.
